The most common tool offered for this is botulinum toxin injected into the masseter, the thick chewing muscle that sits over the angle of the jaw. The treatment is real, the mechanism is well understood, and the results can be genuinely visible. But it only works when the width of the face is coming from muscle in the first place, and that is a detail marketing often skips.

How the mechanism actually works

The masseter is one of the strongest muscles in the body relative to its size. In people who clench, grind, or chew gum heavily, it can hypertrophy the same way a bicep grows with training. Botulinum toxin blocks the release of acetylcholine at the neuromuscular junction, which reduces the muscle's ability to contract at full force. A muscle that contracts less begins to atrophy, meaning it shrinks in volume over weeks. That volume loss is the slimming effect. Nothing is dissolved, removed, or melted. The muscle simply deconditions, the way an arm shrinks after weeks in a cast.

This is why the timeline is slower than typical wrinkle treatment. Most side effects are dose and placement related. Injecting too superficially or too far forward can affect the risorius or zygomaticus muscles, producing a temporarily asymmetric smile. Over treatment can cause chewing fatigue, especially with tough foods, in the first few weeks. A rarer but documented issue is paradoxical bulging, where superficial fibers of the masseter compensate and create a small lump on clenching, usually managed with a small touch up dose.

One longer term consideration is repeated high dose treatment over many years. Because the masseter contributes to the soft tissue support at the jaw angle, aggressive ongoing atrophy in a patient with thin skin and low facial fat can, in some cases, contribute to a hollowed or saggy look at the lower cheek.
