Both procedures address a recessed or short chin, but they work through entirely different mechanisms, carry different risk profiles, and suit different anatomies. Here is what the evidence and surgical literature actually say.

A weak or recessed chin, what clinicians call microgenia or retrogenia, is one of the most common reasons patients seek lower face contouring. It shortens the visual length of the jawline, blunts the transition from chin to neck, and can make a balanced nose look disproportionately large in profile. Two mainstream options exist: injectable filler and a surgical implant. They are frequently marketed as interchangeable. Mechanically, they are not.

How filler works at the chin

Most chin augmentation with filler uses high G-prime hyaluronic acid or, less commonly, calcium hydroxylapatite. G-prime is a measure of a gel's stiffness, its resistance to deformation under pressure. The chin sits at the pogonion, a point subjected to constant force from the mentalis muscle, which contracts every time you speak, chew, or purse your lips. A soft gel placed here spreads and flattens. A stiff gel resists that force and holds projection. This is why practitioners typically inject firm products in small supraperiosteal boluses, meaning directly on the bone, deep to the muscle, where the gel behaves most like a structural support and least like a soft tissue layer.

Filler at the chin generally lasts 12 to 24 months, longer than in mobile areas like the lips, because the deep plane is relatively protected from mechanical churn. Typical volumes run from 1 to 4 milliliters depending on the deficit. Results are adjustable and, with hyaluronic acid, reversible using hyaluronidase, an enzyme that dissolves the gel.

How an implant works

A chin implant is a preformed piece of solid silicone or porous polyethylene placed through a small incision, either under the chin or inside the lower lip, into a pocket created directly on the mandible. Silicone implants become surrounded by a thin fibrous capsule and can be removed later if needed. Porous polyethylene allows some tissue ingrowth, which stabilizes the implant but makes removal more involved.

The key mechanical difference: an implant provides fixed skeletal-level projection that does not degrade, migrate under muscle load, or require maintenance. It also comes in shapes that can extend laterally along the jawline, addressing prejowl hollowing in a way that would require significant filler volume to replicate.

Where each option genuinely excels

Filler is well suited to mild to moderate deficits, patients who want to trial a new profile before committing, and cases where asymmetry needs fine adjustment. It requires no anesthesia beyond topical or local numbing and no recovery beyond a few days of swelling and possible bruising.

An implant makes more sense for moderate to severe retrogenia, patients tired of repeat injection costs, and those who want durable lateral jawline support. Over a decade, cumulative filler costs at the chin frequently exceed the one-time cost of surgery, a point worth calculating honestly rather than assuming filler is the budget option.

A third category matters here: if the chin deficit comes with a significant dental occlusion problem, meaning the bite itself is off, neither filler nor a standard implant addresses the underlying skeletal relationship. Those patients belong in an orthodontic or orthognathic surgery consultation first.

Risks, stated plainly

Filler risks at the chin include lumpiness, asymmetry, and the rare but serious complication of vascular occlusion. The mental artery and its branches exit the mandible near the injection zone, and gel entering a vessel can compromise skin. Deep, slow, low-pressure boluses on bone reduce but do not eliminate this risk. There is also emerging attention on long-term bone effects: some imaging studies suggest repeated deep filler placement may be associated with localized bone remodeling, though the clinical significance remains under study.

Implant risks include infection, implant shifting, temporary or rarely permanent numbness of the lower lip from mental nerve irritation, and the general risks of anesthesia. Long-term studies of silicone chin implants show high satisfaction rates, but revision surgery, while uncommon, is more involved than dissolving filler.

The myth worth retiring

A persistent claim holds that filler is always the safer choice because it is nonsurgical. Safety is not a single axis. Filler carries a small vascular risk at every session, and sessions repeat for life if you want to maintain the result. An implant concentrates its risk into one surgical event. Neither is categorically safer, they distribute risk differently over time.

Bottom line

Ask any injector or surgeon one question: how many milliliters, or how many millimeters of projection, does my deficit actually require? Small numbers favor filler. Large numbers, lateral jawline involvement, or fatigue with maintenance favor an implant. And if your bite is part of the problem, start with a dental workup, not a syringe.
