Masseter Botulinum Toxin vs Buccal Fat Removal: Two Very Different Routes to a Slimmer Lower Face
Both procedures are marketed as face slimming, but they work on different tissues, suit different anatomy, and carry very different long-term stakes. Here is how clinicians actually decide between them.

Ask three patients why their lower face looks wide and you may get three different answers from an examining clinician: muscle, fat, or bone. That distinction matters, because the two most requested slimming procedures, masseter botulinum toxin injection and buccal fat pad removal, target completely different structures. Choosing the wrong one for your anatomy means spending money on a treatment that cannot deliver the change you want.
What each procedure actually does
The masseter is one of the primary chewing muscles. It runs from the cheekbone down to the angle of the jaw, and in some people it hypertrophies, meaning it enlarges from heavy use. Chronic clenching, bruxism, gum chewing, and genetic predisposition all contribute. When the masseter is bulky, the face takes on a squared, wide appearance at the jaw angle. Botulinum toxin works here the same way it works everywhere: it blocks acetylcholine release at the neuromuscular junction, reducing the muscle's ability to contract at full force. Over roughly six to twelve weeks, the partially inactivated muscle atrophies and the jaw angle visibly narrows. The effect is gradual, reversible, and typically lasts four to six months before retreatment is needed. Many patients also report reduced clenching and jaw tension, which is why the same injection pattern is used off label for bruxism.
Buccal fat removal is surgery. The buccal fat pad is a discrete, encapsulated fat compartment sitting deep in the mid to lower cheek, between the buccinator muscle and the masseter. A surgeon makes a small incision inside the mouth, teases out a portion of the pad, and removes it permanently. The result is a hollowing of the area below the cheekbone, which can sharpen the transition between the cheekbone and the lower face. Critically, this procedure does nothing to the jaw angle itself. It changes cheek fullness, not jaw width.
Matching the procedure to the problem
A reasonable clinical exam sorts patients quickly. If the width is at the angle of the jaw, and the bulk firms up when the patient clenches their teeth, the masseter is the driver and toxin is the logical tool. If the fullness sits in the mid cheek, feels soft, does not change with clenching, and persists at a healthy body weight, the buccal fat pad may be contributing. Some patients have both issues. Others have neither: a wide mandibular bone or generalized facial adiposity tied to body weight, in which case neither procedure will produce the expected result.
One caveat clinicians raise repeatedly: buccal fullness in a person in their early twenties is often normal facial fat that will diminish on its own. Facial fat compartments deflate with age, and the buccal region is no exception. Removing that fat surgically at 23 can look sculpted at 30 and gaunt at 45, because the pad cannot be put back. This is the central risk asymmetry between the two procedures. Toxin wears off. Fat removal does not.
Risk profiles, in plain terms
Masseter toxin has a short but real list of complications. Overtreatment or poor placement can weaken chewing strength temporarily, and diffusion into nearby muscles such as the risorius or zygomaticus can cause an asymmetric smile until the toxin wears off. Injecting too superficially or too anteriorly raises that risk, which is why injectors stay within the muscle's safe zone, typically the lower posterior portion. Repeated treatment over years produces sustained muscle thinning, and some patients extend their retreatment intervals over time.
Buccal fat removal carries surgical risks: infection, hematoma, asymmetry, and injury to the parotid duct or buccal branches of the facial nerve, both of which pass close to the operative field. Competent surgical technique makes these uncommon, but the more discussed long-term concern is aesthetic: premature facial hollowing as natural age-related fat loss stacks on top of surgical removal. Conservative surgeons now remove less tissue than was fashionable a decade ago, and many decline the procedure entirely in patients with lean faces or a family history of gaunt aging.
Cost and commitment logic
Toxin is cheaper per session but recurring, often two to three sessions per year to maintain results. Buccal fat removal is a one-time cost, higher upfront, with a permanent outcome for better or worse. Framed as a decision rather than a price comparison: the reversible option lets you audition a change, the permanent option requires you to be confident about how your face will age.
The bottom line
These are not competing versions of the same treatment. Masseter toxin narrows a muscular jaw angle and is reversible. Buccal fat removal hollows the mid cheek and is permanent. A proper assessment starts with a clench test and an honest conversation about age, weight stability, and how faces lose volume over decades. If a provider recommends either procedure without examining which tissue is actually responsible for the fullness, that is a reason to seek a second opinion.
