Masseter Botulinum Toxin vs Buccal Fat Removal: Two Very Different Routes to a Slimmer Lower Face
Both are marketed as face slimming, but one shrinks a muscle and the other removes a fat pad. Understanding the anatomy behind each explains who benefits, who does not, and why the results age differently.

Search interest in lower face slimming has grown steadily, and two procedures dominate the conversation: botulinum toxin injections into the masseter muscle and surgical removal of the buccal fat pad. They are often discussed as interchangeable, but they act on entirely different structures. Choosing between them without understanding that distinction is where most patient disappointment begins.
What each procedure actually targets. The masseter is one of the primary muscles of chewing. It sits over the angle of the jaw, and in some people, particularly those who clench or grind their teeth, it hypertrophies, meaning it grows larger with use, the same way a biceps grows with training. Injecting botulinum toxin into the masseter blocks acetylcholine release at the neuromuscular junction, reducing the muscle's activity. Over roughly six to twelve weeks, the underused muscle atrophies and shrinks, narrowing the lower face at the jaw angle. The buccal fat pad, by contrast, is a discrete, encapsulated collection of fat that sits deep in the mid cheek, between the buccinator muscle and the masseter. Removing part of it through a small incision inside the mouth hollows the area below the cheekbone, creating a more sculpted or concave contour in the lower mid face.
Why the same complaint can have different causes. A patient who says my face looks wide or round may have masseter hypertrophy, prominent buccal fat, generalized facial fullness from body composition, low skeletal definition at the jaw, or some combination. Palpation is the simplest sorting tool: a clinician asks the patient to clench, and if the sides of the jaw bulge firmly under the fingers, muscle is contributing. If the fullness is soft, sits higher and more forward in the cheek, and does not change with clenching, fat is more likely responsible. Imaging is rarely needed, but a careful physical exam is, and it is the step most often skipped when procedures are sold rather than prescribed.
Reversibility and the arc of results. Masseter injections are temporary. The toxin's effect fades over three to six months, and the slimming effect fades more slowly, often lasting six to twelve months after a few consecutive treatment cycles because the muscle takes time to rebuild. If a patient dislikes the result, stopping treatment allows recovery. Buccal fat removal is permanent. The fat pad does not regenerate, and this is both the appeal and the risk. The buccal fat pad naturally deflates with age in many people. A face that looks pleasantly sculpted at 25 after fat removal can look gaunt or skeletonized at 45 when age related volume loss compounds the surgical deficit. This is the core reason many surgeons now counsel conservative removal or decline the procedure in patients with naturally lean faces.
Side effect profiles differ in kind, not just degree. Masseter toxin risks include temporary chewing fatigue, asymmetric smile if the toxin diffuses into nearby muscles such as the risorius or zygomaticus, and in rare cases with repeated high doses, changes in bite force. These effects resolve as the toxin wears off. Buccal fat removal carries surgical risks: injury to the buccal branch of the facial nerve, which controls some movements of the mouth, injury to the parotid duct, which carries saliva and runs directly through the operative field, infection, hematoma, and asymmetry that is difficult to correct because you cannot put the fat back. None of these are common in experienced hands, but they are structural risks rather than temporary ones.
Myth check: buccal fat removal defines the jawline. It does not, at least not directly. The buccal fat pad sits above and forward of the jawline. Removing it hollows the cheek and can make the cheekbone and jaw appear more prominent by contrast, but it does not sharpen the mandibular border itself. Patients seeking jawline definition specifically are usually better evaluated for skeletal support, skin laxity, or submental fat, each of which calls for different tools.
Myth check: masseter toxin will make the face sag. In typical cosmetic doses this is unlikely. The masseter contributes little to soft tissue support of the mid face. However, in older patients with significant skin laxity, reducing the underlying muscle volume can occasionally unmask looseness that was being propped up, which is why age and skin quality belong in the consultation.
The practical takeaway. If the fullness clenches, think muscle and consider toxin, which is adjustable and reversible. If the fullness is soft, deep, and mid cheek, the buccal fat pad may be involved, but permanence demands caution, especially in patients under 30 whose faces have not yet begun natural volume loss. A trial of the reversible option first, or simply waiting, is often the most defensible clinical advice, even if it is the least exciting to market.
